The LVN is expected to keep accurate and up-to-date documentation of patient care and progress notes. Performing routine assessments and working collaboratively with the healthcare team to achieve the best patient outcomes are key. The position also entails responding quickly to emergencies by providing the necessary interventions and ensuring adherence to all relevant regulations and company policies.

Applicants must hold an active California LVN license in good standing and have at least one year of nursing experience, preferably in a home health setting. A current CPR/BLS certification is required. The role demands proficiency in wound care, IV administration, and medication management. Strong interpersonal and communication skills are essential. Candidates must have a valid California driver’s license, reliable transportation, and the ability to work independently and manage time efficiently. Experience with electronic medical records (EMR) is considered a plus.
