Our hospice is seeking an EXPERIENCED Intake Coordinator. A strong command of the English language is essential for this role.
Responsibilities include:
• Coordinating the entire process from Admission to Discharge of the patient using HospiceMD
• Screening clients for eligibility for direct and support services, obtaining orders, and referring clients to necessary services
• Scheduling nurses for patient care
• Maintaining comprehensive patient documentation
• Managing client information, schedules, files, and forms with a high level of confidentiality
• Liaising with doctor's offices and hospitals
• Conducting wellness follow-up checks with patients
• Performing other related duties as assigned
