We're in search of a Home Health Care Quality Assurance (QA) Specialist with a keen eye for detail and exceptional organizational skills. This vital role involves collaborating with clinicians, administrators, and compliance officers to maintain high standards of documentation, align with regulatory requirements, and enhance the quality of patient care.
Key Responsibilities
Documentation Review & Compliance
You'll be tasked with auditing and reviewing patient documentation, visit notes, care plans, and physician orders to verify their accuracy and compliance with home health regulations. It's crucial that all documentation adheres to Medicare, Medicaid, state, and agency-specific guidelines. Identifying and rectifying any discrepancies in documentation will also fall under your purview, as will keeping abreast of the latest in home health care regulations and best practices.
Quality Assurance & Process Improvement
Your role will involve conducting internal audits to pinpoint areas for improvement, offering feedback and training on documentation best practices, and spearheading quality improvement initiatives to boost documentation efficiency and compliance. Ensuring adherence to patient care and documentation policies and procedures is also key.
Collaboration & Reporting
A significant part of your role will be working closely with clinical teams, compliance officers, and agency leadership to uphold quality standards. You'll prepare detailed audit reports, recommend corrective actions, and assist with regulatory surveys and accreditation processes by ensuring documentation readiness.
Training & Support
Educating and guiding home health care staff on proper documentation practices, developing and updating compliance and quality assurance educational materials, and supporting new hires in understanding documentation protocols and regulatory requirements are crucial tasks in this role.
Qualifications & Requirements
Education: A degree in Nursing, Healthcare Administration, or a related field is preferred.
Experience: Candidates should have at least 2-3 years of experience in home health care, hospice, or a similar healthcare setting, with a strong preference for those with experience in quality assurance, auditing, or compliance related to home health care. Previous experience as a Home Health QA Specialist is essential.
Certifications: Having a Certification in Home Health Compliance (CHHC) or related credentials will be considered an asset.
Technical Skills: Proficiency in Electronic Medical Records (EMR) systems such as HCHB, Kinnser, WellSky, Axxess, and a thorough understanding of Medicare, Medicaid, and ACHC/JCAHO home health care regulations are required.
Key Competencies: We're looking for someone with an excellent attention to detail, strong analytical, communication, and training skills, the ability to work both independently and as part of a team, and proficient problem-solving abilities to identify and address compliance risks.
