Join Our Team as an Intake Coordinator 
   An esteemed Home Health Agency is actively seeking a dedicated individual to fill the role of intake coordinator on a full-time basis. The selected candidate will be responsible for a variety of tasks including verifying patient eligibility, managing schedules, posting updates, handling incoming calls and emails, and maintaining continuous communication with patients. 
     Work Schedule: Monday to Friday, 9:00 AM to 4:00 PM 
   Employment Type: W-2, Full-Time 
   Requirements: 
    -  Minimum of one year experience in home health care, Synergy experience preferred 
   -  Fluent in English with a valid work permit 
   -  Exceptional communication, interpersonal, and organizational abilities 
   -  Ability to prioritize tasks effectively and work collaboratively within a team
