Join Our Team: Seeking Experienced Intake Coordinator
Our home health service is on the search for an experienced Intake Coordinator. The role involves:
- Scheduling the entire patient care journey from Admission to Discharge using KINNSER.
- Screening clients for eligibility for both direct and support services, obtaining necessary orders, and referring clients to essential services such as physical therapy, occupational therapy, speech therapy, social workers, and mental health services.
- Scheduling nursing staff.
- Maintaining comprehensive patient documentation.
- Tracking client information, schedules, files, and forms with a high degree of confidentiality.
- Facilitating communication between doctor's offices, hospitals, and the care team.
- Conducting wellness follow-up checks with patients.
- Handling other related assignments as needed.
Requirements: Applicants must have prior experience in a similar role and must be fluent in English.
